What the Aflac Dental PPO Plan 2 Covers

Preventive Care (100% Covered)

Two cleanings and exams per year are fully covered with no deductible. Bitewing x rays, full mouth x rays, sealants for kids under 16, and fluoride treatments for kids under 19 are included.

Basic Services (80% Covered)

Fillings, simple and surgical extractions, emergency palliative treatment, and crown or bridge repairs are covered at 80% after your deductible.

Major Services (50% Covered)

Crowns, bridges, dentures, implants, periodontal treatment, endodontics, oral surgery, and anesthesia are all covered. Dental implants are included, something a lot of plans skip entirely.

A Deductible That Gets Smaller Every Year

Year 1 Deductible: $50 per person (up to 3 per family)

Year 2 Deductible: $25 per person (up to 3 per family)

Year 3 and Beyond: No deductible at all

Annual Maximum Benefit: $1,500

Maximum Carryover Benefit: Up to $1,000 additional
Members earn $250 in carryover each year they stay under $500 in paid claims and submit at least one preventive claim. Unused carryover rolls forward and can be used for any covered procedure.

The deductible starts at just $50 per person in year one. It drops to $25 in year two. By year three, there is no deductible at all.

Finding a Dentist Is Simple

This is an Aflac PPO plan, which means you have access to a wide network of participating dentists. You can also see a dentist outside the network. You will just pay a bit more out of pocket for out of network visits.

To find a participating provider near you, visit aflac.com/DentalNetwork and use the provider search tool. When you schedule your appointment, let them know you are an Aflac Dental PPO member and bring your ID card.

If you already have a dentist you love, there is a good chance they are in the network. And if they are not, you can still use them. The plan will still pay based on reasonable and customary rates for your area.
